Clinical Care Pathways is the module that turns a policy library into something a nurse can use at three in the morning. It contains two distinct things. The care plan templates are the long-form documents that describe how one person’s condition is managed day to day — what good looks like for this resident, what the early warning signs are, and who to call. The clinical pathways are short, sequenced, decision-first documents for the moments where minutes matter: sepsis, anaphylaxis, choking, hypoglycaemia, a suspected stroke.

Both are written against NICE guidance and, where NICE is silent for a care home setting, against the relevant specialist body — the Resuscitation Council UK, the UK Sepsis Trust, Diabetes UK. Clinically they support Regulation 12, because safe care and treatment is what an inspector is testing when they read a care plan. Evidentially they support Regulation 17, because a care plan that is written, followed and reviewed is the clearest governance evidence a home can produce. They are useful in residential as well as nursing homes: where staff cannot deliver a clinical intervention, the pathway becomes the escalation prompt that gets the right professional there faster.
